What to Bring to Your Appointment
If you are a first-time patient, please plan on arriving early to complete a patient registration form. (You can download the forms and fill them before) Please bring the following items to the appointment:

Insurance card(s)

Referral forms (if required)

List of all prescription and over-the-counter medications that you are currently taking

Any required co-payment

Pertinent medical and surgical history information

X-ray films that you may have

Filled New Patient Information Form (see below to download)

You can download , fill and print the interactive "new patient form" for your appointment using the links to the PDF document below. You can bring it to your appointment or fax it to the office.

Appointments
Appointments are available online or by calling our office.
Please call the office to make an appointment in advance. If you're unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we're are late due to emergencies or hospital surgery.
Insurance and Payment Information
We encourage all patients to confirm their coverage prior to making an appointment. We accept most major Insurance plans. Co-payments are due at the time of the appointment. You are responsible for any unpaid balance not covered by insurance. Please call us with any questions regarding which insurance plans we accept or other billing concerns. Insurance coverage and benefit questions should be directed to your health insurance provider.
Pre-Operative Instructions
You must arrive at the hospital at least 2 hours prior to your scheduled surgery time.
